Instructions for Downloading and Installing the Fitness Application

NOTE This program is NOT COMPATIBLE with Microsoft VISTA operating systems or Macintosh operating systems. |
This version must be downloaded on a computer which:
1. has a drive to which you can save your answers (i.e. 3.5 diskette, CD or other external storage medium)

You must save your answers by clicking on “Save Answers to an External Storage Medium” from the Main Menu of the installed application. The medium to which you save your answers must be labeled with your full name and submitted with your application. |
2. has a printer installed

you must click on “Print Application” from the Main Menu of the installed application and submit the entire printed application INCLUDING (1) a signed and notarized Continuing Application and Verification and (2) a signed an notarized Authorization and Release |
When downloading the program, depending on your browser, you may be given different options. You may choose to download the “Setup.exe” file to your own computer and run it from there or you may choose to install the Fitness application directly from the Internet without downloading any files to your computer first (some browsers do not support this option).
To download the program to your computer and install it . . .
1. Click on the button to begin download
2. Choose the option to “Save this program to disk”.
3. The downloadable file is called “Setup.exe”. Choose a place to save the file where you can find it later. Usually the desktop is the most practical place to save the file.
4. Find the file you have just saved (after it has finished downloading) and double click it to start the installation.
5. Continue with the Installation Instructions below.
To install the program directly from the Internet . . .
1. Click the button to begin download.
2. Choose the option to “Run this program from its current location”.
3. You may be given a security warning (this will not happen on al browsers Click “Yes”

Installation Instructions
1. You may choose all of the defaults by clicking “Next” 3 times, then by clicking “Finish”. The program will automatically be installed in your Windows “Program Files” directory unless you specify otherwise.
2. A shortcut will be placed on your desktop and also an icon will be placed in your Start Menu under Programs >Fitness Application. Choose either one of these shortcuts to start the Fitness Application Install.
3. Upon successful installation of the Fitness application, you will no longer need the “Setup.exe” file. If you wish, you may delete it from your system.

Warning: If you run the “Setup.exe” file a second time (either from the desktop or from the Internet) after beginning to fill out the Fitness application, all of your data will be overwritten. For this reason, it is recommended that you delete the “Setup.exe” program from your computer after installation. You will only need to re-install the application in the event of a fatal system error or if directed to do so by Technical Support. |
