Two fingerprint cards are required when submitting the Fitness Application.
YOU MUST USE CARDS OBTAINED FROM THE OFFICE OF BAR ADMISSIONS. The agencies to which fingerprint cards are submitted for processing in Georgia will not accept cards which are obtained from any other agency, jurisdiction or police department.

Complete this request form. Two fingerprint cards will be mailed to you within 1-2 business days. |
You must have your fingerprints “rolled” on these cards by an authorized agent (police department, sheriff’s department, business, Foreign Embassy, etc.). The agent must sign both cards and provide the agency name on the cards. You must sign and complete all applicable identifying on both cards.
When completing the identifying information, please note that the processing agencies require the use of the following abbreviations only:
