It is the applicant's responsibility to request that the appropriate authorities send transcripts and the MPRE score to this office.  Please submit this form only after you have contacted the authorities and allowed sufficient time for the documentation to be received by this office.

NOTE:  If you have received written notification that your Bar Examination Application is complete, that means that the required documentation has been received.   Please do not submit this form.
