, hereby consent to allow the Georgia Board to Determine Fitness of Bar Applicants, including its agents and representatives (hereinafter collectively the “Board”), to conduct an investigation as to my moral character, professional reputation, and fitness for the practice of law in the State of Georgia. I further authorize and request every school official and every other person, firm, officer, corporation, association, organization, or institution having control of any documents, records, or other information pertaining to me relevant to my character and fitness to furnish copies of any such documents, records, and other information to said Board. Additionally, I authorize the Board to inspect and make copies of such documents, records, or other information.
I hereby authorize all such persons as set out above to answer any questions concerning the undersigned that may be submitted to them by the Board, and to appear before said Board, and to give full and complete testimony concerning the undersigned, including any information furnished by the undersigned. I hereby relinquish any and all rights to reports or other information incident in any way to their cooperation with the Board and fully understand that I shall not be entitled to have disclosed to me the contents of any of the foregoing.
I hereby release and exonerate every school official and every other person, firm, officer, corporation, association, organization, or institution that complies in good faith with the authorization and request made herein from any and all liability of every nature and kind growing out of or in any way pertaining to the furnishing or inspection of such documents, records, and other information or the investigation made by the Board. I further waive absolutely any privilege I may have relevant to my good moral character and fitness to perform the responsibilities of an attorney.
For the purpose of this release, I give permission to use a copy of my electronic signature on this form as an original signature.
In connection with the investigation into my character and fitness and in accordance with the Board's Policy Statement on financial responsibility, I hereby authorize the Board to obtain a credit report from a consumer reporting agency, which will generate a
hard inquiry that could have a minor impact on my credit score.

I also understand that I will only be entitled to a refund of two-thirds of the application filing fee (excluding any late fee) upon providing a written withdrawal request within 15 business days of the submission of my application to the Office of Bar Admissions.
