First Name:	You must supply your First Name. First Name must be less than 64 char.
Middle Name:	If you do not have a middle name, you must check the box below. I do not have a middle name. You must supply your Middle Name. Middle Name must be less than 64 char.
Last Name:	You must supply your Last Name. Last Name must be less than 64 char.
Suffix:	Suffix must be less than 12 char. Suffix must be alphanumeric characters.
Social Security Number:	ssn1 ssn2 ssn3 If you do not have a social security number, you must check the box below. no_ssn I do not have a social security number.
Date of Birth: Month Day Year	This is not a valid Date of Birth.
Place of Birth:	You must supply your place of birth.

Law School Id Law School: Please be aware that our Rules require graduation from an ABA-approved law school.
NCBE Number:	You must supply your NCBE number. NCBE Number must be less than 64 char.
